Signs you may need root canal treatment

• Throbbing or spontaneous toothache, especially at night
• Pain on biting or chewing
• Sensitivity to hot or cold that lingers after the trigger is removed
• A darkened or discoloured tooth
• Swelling or a pimple like bump on the gum near the tooth
• A deep cavity, a cracked tooth or a large old filling that has started to ache

Not every toothache needs a root canal. Some of these signs overlap with other problems, so the first step is always an examination and an X-ray to confirm what is actually going on. If the nerve is healthy, we tell you so and treat the real cause instead.

What happens during root canal treatment

Root canal treatment at our clinic usually takes two to three visits, depending on the tooth and the extent of infection.

Visit one, relieving the pain and removing the infection. The tooth is numbed with local anaesthetic. A small opening is made in the top of the tooth, and the inflamed or infected pulp is removed. A medicated dressing is placed inside the tooth and it is sealed temporarily. For most patients, the pain settles significantly within a day or two of this visit.

Visit two, disinfection and shaping. The temporary seal and dressing are removed, and the working length of each canal is measured precisely. The canals are then cleaned and disinfected with an antibacterial irrigation solution and shaped with fine instruments to prepare them for the final filling. If the infection was severe, a fresh medicated dressing is placed and the tooth is given another one to two weeks to settle fully.

Visit three, sealing the tooth. Once the tooth is calm and symptom free, the canals are filled with a rubber like material called gutta percha and sealed to prevent reinfection. The tooth is then restored with a filling.

Afterwards, protecting the tooth. A back tooth that has had root canal treatment becomes brittle over time, so a crown is usually recommended to protect it from cracking. We will advise you if your tooth needs one. You can read more on our dental crowns page.

Front teeth have one canal and are the most straightforward. Molars have three or four canals and take longer, which is also why fees differ by tooth type.

Does root canal treatment hurt?

This is the question we hear most. The honest answer: the treatment itself is done under local anaesthetic, and most patients tell us it felt no different from having a filling done. The pain people associate with root canals is usually the toothache before treatment, not the procedure itself. Mild tenderness for a few days afterwards is normal and typically manageable with ordinary painkillers.

Root canal or extraction?

When a tooth is badly infected, the choice is usually between root canal treatment and extraction. Points worth weighing:

Keeping the tooth. A root treated and crowned tooth lets you chew naturally and keeps the teeth beside it in place. Nothing artificial matches the function of your own tooth.

What replacement costs. Extraction is cheaper on the day, but replacing the missing tooth later with an implant or bridge costs considerably more than the root canal would have. Leaving the gap unfilled lets neighbouring teeth drift and bite problems develop.

When extraction is the right call. If a tooth is cracked below the gum, has too little structure left to restore, or has severe gum disease, extraction may genuinely be the better option. We will tell you honestly which situation you are in.

Visiting endodontist for complex cases

Some root canal cases need more than standard treatment: unusually curved or narrow canals, teeth where previous root canal treatment has failed and needs redoing, or persistent infection that does not settle. For these cases, a visiting endodontist, a dentist with specialist training in root canal treatment, is available at our clinic.

Aftercare following root canal treatment

• Avoid chewing on the treated tooth until the final restoration or crown is placed
• Mild soreness for two to three days is normal, ordinary painkillers are usually enough
• Contact us if you experience swelling, severe pain or a broken temporary filling
• Complete the crown if one is recommended, an unprotected back tooth can crack
• Continue normal brushing and flossing, a root treated tooth still needs care
